Office Policies

To Our Dear Patients,

PLEASE HELP US TO CONTINUE PROVIDING YOU WITH THE HIGHEST QUALITY, MOST COST EFFECTIVE AND CARING DENTAL SERVICES BY FAMILIARIZING YOURSELF WITH AND ADHERING TO OUR OFFICE POLICIES:

NO SHOWS/APPOINTMENT CANCELATIONS

Please remember to cancel your appointment with us at least 24 hours in advance by calling the office during office hours.  If you do not cancel and/or do not show up for your appointment, it means we end up with an empty time slot that we could have dedicated to another patient.  We will permit a single missed appointment in a calendar year.  However, if you miss more than one appointment in a calendar year without informing us 24 hours in advance, your subsequent bill will include the appointment no show cancelation fee of $50 to $100 (fee depends on the length of your originally scheduled appointment).

ADDITIONAL SERVICES

You will be notified while you are with the dentist and/or hygienist if there is a need for them to carry out any additional treatment not discussed previously when you originally made your appointment. Although not advised, you have the right to refuse any recommended/required treatment prior to it being undertaken. 

OUTSTANDING BALANCES/COLLECTIONS

Balances are due upon your receipt of invoice. Accounts will be forwarded to collections if balances are outstanding for up to 90 days. Additionally, we may have to delay further service until balances are made current. If you have any queries regarding your co-pays and dental coverage, kindly contact your insurance provider for an explanation of your plan’s benefits.

BILLING

You may receive a bill for services from City Place Dental and/or your insurance provider.  It may take up to 30 days for City Place Dental to invoice you following your visit with us depending on what part of the billing cycle your visit falls into. Invoices generated by your insurance provider have different billing cycles that are not generally known by City Place Dental

CO-PAYS

All co-pays are set by your individual insurance plan and are due at time of service.

The nature of our practice is to give our patients the utmost in care and service therefore please excuse any delays. We will give you the same careful attention. Likewise, it is imperative that you arrive on time for your appointment so that you don’t hold up fellow patients & staff and to also ensure that you don’t forfeit your own scheduled appointment with us. Should you have any questions or concerns, please let one of us know. We will do all we can to resolve your worry.
